There are several methods to create banners. I'd say each banner maker has its own way of doing them. What I will try to explain with this tutorial is how to blend various pictures together, in order to produce one single image. This technique is extremely easy, and is intened for people with no experience. The software I'll be using is Photoshop 7.0. (Thanks to The Demon's Jumble for the pics and to ImageShack for the Free Image Hosting.)
Step 1: Open up your Photoshop.
Step 2: Create a 400x100 canvas. Go to File/New.
Step 3: Open up all the images you want to blend together
Step 4: Choose the square marquee tool.
Step 5: Use the square marquee tool to select a square in one of the pictures.
Step 6: Go to select/feather, change the feather radius to 20 and then click ok.
Step 7: Go to edit/copy
Step 8: Go to your blank canvas and go to edit/paste.
Step 9: Go to edit/transform/scale and scale down your image. Position it as you like.
Step 10: Now select, feather, copy, paste,resize and position your other images.To save it, go to layer/flatten image and then to file/save as (make sure you choose the JPEG option under the second scrollable bar)
